I don't have MPLAB X IDE v4.05 installed at the moment to double-check but looking at the release notes I see it says the PIC16F716 is not supported by the MPLAB ICD 4 tool yet. http://www.microchip.com/mplabx-ide-release-notes If your MPLAB ICD 4 tool is stuck in Bootloader mode a quick way to flash in the normal Application tool firmware is to: 1) Launch the MPLAB IPE v4.05. 2) Select a supported part, say, PIC16F1939. 3) Select the MPLAB ICD 4 tool. 4) Click 'Connect' The firmware should be updated and both LEDs should turn blue.
